Description:
Meet with Persons Served (PS)/Family in their homes to evaluate and assess the needs of the PS.
Schedule timely follow up visits to ensure provision of services by the assigned Direct Support Professional (DSP).
Bring and complete all necessary paperwork to the home visit with the PS.
Relaying PS information to the Coordinator and the Program Director.
Conducts training and development programs to the DSP in the courses needed for qualification to provide care in the home setting and maintains documentation of training received
